In August of 2012, the state of Vermont launched the Pediatric Palliative Care Program (PPCP) to address the physical, emotional, and psychosocial needs of children with life-limiting/life-threatening conditions (LL/LTC). The PPCP was made possible through the Vermont Chronic Care Initiative and Integrated Family Services, as well as an amendment to Vermont’s 115 Demonstration Waiver. However, in contrast to many other states, Vermont allows children with LL/LTC to not only receive concurrent care but also allows children to qualify for hospice care without the determination of a six-month life expectancy.
